HEALTH INFORMATION SERVICES (MEDICAL RECORDS)

The Department of Health Information (Medical Records Department) is responsible for the management and processing of all patient care data. This department functions between the hours of 7.30 am - 4.30 pm on weekdays.

The main functions include:

1. Coding and abstracting of patient's diagnoses and operations using IDC-10cm coding.

2. Release of medical information upon signed authorization from the patient. Please note that consent of the patient is not required when a subpoena is served which mandates that records be produced in court.

3. Medical transcription of patient histories, discharge summaries, medical reports, etc.

4. Preparation of medical abstracts for Insurance Companies. A fee is payable to the Tobago Regional Health Authority (TRHA) for preparation of the reports. Requests must be made one week in advance.
- National Insurance forms - $15.00
- Casualty Reports - $22.50
- In-patient Reports - $37.50
- Consultant Reports - $75.00

5. Preparation of In-patient and Out-patient statistical data.

6. Documentation of Births and Deaths which occur in the hospital.

7. Making appointments for Outpatient Clinic and Medical Board Examinations.
